The University of Hawai'i Sports Medicine Fellowship participates in the primary care sports medicine fellowship match through the National Residency Matching Program (NRMP). Applicants must be Board-certified or Board-eligible in Family Medicine, Pediatrics, Physical Medicine and Rehabilitation, or Internal Medicine. Applicants must have passed Step 2 of the USMLE exam. Since applications are reviewed on a rolling basis, early application is strongly encouraged.
Applications will be accepted for the 2017-2018 year through ERAS in July!

Applicants may submit the Universal Application for Residency available from the National Resident Matching Program (NRMP) through ERAS. When using the NRMP application, please attach an addendum including residency location, dates and specialty, as well as Sports Medicine rotations, coverage, and conferences attended/presented.
Additional application materials to be submitted include:
- Three letters of recommendation from physicians or other professionals who can attest to applicant's clinical, research, and/or teaching skill and interest
- Curriculum vitae
- Medical school dean's letter
- Copies of medical school diploma, medical license, and validation of residency
- Personal statement should be no longer than one (1) page in length and address: (a) career plans/goals; (b) expectations of fellowship experience; (c) reason for specific interest in University of Hawai'i program; and (d) teaching and research experience.
