This is a summary of the annual report of the Hawaii Residency Programs Employee Benefit Program, EIN 99-0215841, Plan No. 501, for period July 1, 2006 through June 30, 2007. The annual report has been filed with the Employee Benefits Security Administration, U.S. Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

Insurance Information
The plan has contracts with The Lincoln National Life Insurance Company, Unum Life Insurance Company of America and Hawaii Medical Service Association to pay life, accidental death & dismemberment, medical, dental, vision and long-term disability claims incurred under the terms of the plan. The total premiums paid for the plan year ending June 30, 2007 were $1,398,196.
