Aloha and welcome to the University of Hawai‘i Sports Medicine Fellowship Program!
Our 12-month, ACGME-accredited fellowship provides board certified/board eligible physicians in Family Medicine, Internal Medicine, and Pediatrics with additional training in the academic discipline of Sports Medicine. Successful completion of the fellowship leads to eligibility for Board Certification (CAQ) in Sports Medicine. The Program is affiliated with the John A. Burns School of Medicine Department of Family Medicine and Community Health (JABSOM-DFMCH) and sponsored by health care partner Hawai‘i Pacific Health (HPH). The sports medicine fellow is an integral part of the athletic health care team at the NCAA Division I University of Hawai’i at Mānoa Athletics Department (UHMAD). Duties include providing medical care to the 500 collegiate student-athletes in 21 sports under supervision of program faculty.
The University of Hawai‘i Sports Medicine Fellowship Program was established in 2002 in affiliation with the JABSOM-DFMCH and original sponsorship of The Queen’s Medical Center. UHMAD Team Physician and John A. Burns School of Medicine (JABSOM) faculty member, Dr. Andrew Nichols, (Head Team Physician 1994-2011), has served as Fellowship Program Director since its inception. Program sponsorship transitioned from Queen’s to HPH in 2013. As of March 2024, the Fellowship Program has graduated 17 sports medicine physicians, with primary specializations in family medicine (8), internal medicine (5), and pediatrics (4). We are gratified that 65% of our graduates have chosen to stay and practice medicine in the state of Hawai‘i.
The Mission Statement of the University of Hawai‘i at Mānoa John A. Burns School of Medicine Sports Medicine Fellowship Program is to train sports medicine physicians to meet the specialty health care needs for the people of the State of Hawai‘i. This includes contributions to the provision of sports and exercise related health care for our Keiki to Kūpuna, the promotion and advocacy of healthy lifestyles, research and scholarly contributions, and the application of physical activity prescription in the clinical care of medical conditions that are prevalent in Hawai‘i and the Pacific region. Our Program commitment extends to and is consistent with the Mission Statements of JABSOM and UH Mānoa.
The program aims of the University of Hawai‘i at Mānoa John A. Burns School of Medicine Sports Medicine Fellowship Program are to:
- Prepare our fellows for a wide diversity of career paths in sports medicine, with a commitment to serving the populations of Hawai‘i and the Pacific Basin.
- Advocate for the health benefits of exercise on a population health basis.
- Utilize evidence-based medicine to optimize patient care for a diverse population of athletes at all performance levels, from our Keiki to Kūpuna.
- Teach and promote sports medicine principles to physicians in training, allied health care providers, our patients, and the community at large.
- Contribute to the body of scientific knowledge in the evolving field of sports medicine.
- Cultivate the practice of lifelong learning in primary care sports medicine.

Your Sports Medicine ‘Ohana
The University of Hawai‘i Sports Medicine Fellowship Program core teaching faculty consists of sports medicine subspecialists with primary specialization in family medicine, pediatrics, internal medicine, orthopaedic surgery, and physical medicine and rehabilitation. Faculty in the fields of radiology, cardiology, exercise science, physical therapy, athletic training, and mental health offer additional instruction. Many of these health professionals provide care for the University of Hawai‘i at Mānoa student-athletes and other high-level athletes.
Program Details
Curriculum
The UH sports medicine fellowship curriculum includes required clinical experiences in sports medicine, athletic training room, orthopedic surgery, radiology/diagnostic imaging, physical therapy/rehabilitation, and the primary medical specialty. The fellow also participates in regular didactic sessions, research, and other scholarly activities. Sporting event and practice coverage is shared with other UH team physicians and faculty.

Research
The UH Sports Medicine fellow is expected to engage in a meaningful research project during the fellowship. This may be achieved by pursuing an individual research project or by contributing significantly to ongoing research by sports medicine faculty. The fellow has access to faculty and university support in research development. The fellow has an average of 1-1.5 half-days of protected research and scholarly activity time per week. The fellow is expected to make every effort to share the findings of their scholarly work with colleagues. This may be in the form of presentation at a national or international academic sports medicine meeting and/or by submission of a manuscript for publication in a peer-reviewed sports medicine professional journal

Training Sites:
- University Health Services Mānoa (UHSM) is an AAAHC-accredited outpatient medical facility providing comprehensive health care for UHM students and the campus community. Medical services include general medicine, women’s health clinics, sports medicine, dermatology, psychiatry, nutrition/dietetics, clinical, pharmacy, substance abuse counseling, and health education.
- University of Hawai‘i at Mānoa Athletics Department Athletic Training Room (UHMAD-ATR) serves approximately 500 student-athletes who participate on 21 varsity teams for the NCAA Division I-FBS The University of Hawai‘i at Mānoa Athletics Department. The athletic training room is staffed by sports medicine physicians (program faculty) and certified athletic trainers.
- Hawai‘i Pacific Health Bone & Joint Centers – Orthopaedic surgery, sports medicine, and musculoskeletal medicine clinics located at Straub, Pali Momi, and Kapi‘olani Medical Centers
Application
The UH Sports Medicine Fellowship participates in the Primary Care Sports Medicine Fellowship Match through the National Residency Matching Program (NRMP). Applicants must be Board-certified or Board-eligible in Family Medicine, Internal Medicine, or Pediatrics. MD applicants must have passed Step 3 of the USMLE exam, and DO applicants must have passed the COMLEX-USA Level 3 exam. Since applications are reviewed on a rolling basis, early application is strongly encouraged.
Applications for the 2025-2026 academic year may be submitted in mid-July 2024 using the Universal Application for Residency available from the National Resident Matching Program (NRMP) through Electronic Residency Application ServiceÒ (ERASÒ).
Additional application materials to be submitted include:
- Three letters of recommendation from physicians or other professionals who can attest to applicant’s clinical, research, and/or teaching skill and interest
- Curriculum vitae
- Medical school dean’s letter
- Copies of medical school diploma, medical license, and validation of residency
- Personal statement should be no longer than one (1) page in length and address: (a) career plans/goals; (b) expectations of fellowship experience; (c) reason for specific interest in University of Hawai’i program; and (d) teaching and research experience.
